Please check the boxes below for the tasks for which you have completed by the prescribed date.

Following are the requirements to earn the Club 100% Secretary Award.
Submit all MMR Reports as required, accurately by the 5th of each following month.

Submit all Club Activity Reports by the 5th of each following month.

PU 101 forms must be submitted to Lions International and District Governor by May 15, 2011

All accounts with Lions International must be paid within 30 days of billing.

All club dues must be paid within 60 days of billing.

Please fill in the appropriate dates for the following items which you have completed.

Attendance at one of the following is required:

A. Three Zone Meetings (which can include a zone social)

B. Two District Cabinet Meetings or other District Training Programs (Includes Fall Leadership Forum)

C. District Convention

Please fill in the appropriate dates for the following items which you have completed.

Attendance is required at one of the following:

A. USA/Canada Forum

B. International Convention

C. Mid-Winter Rally

D. State Council Meeting

E. State Convention
